2.1.3 Medical Entrance Requirements for Students

Policy

Any student born on or after January 1, 1957, whose first-time enrollment for on-campus instruction at ISU occurs on or after July 1, 1989, is required to document proof of immunity against tetanus, diphtheria, rubella, mumps, and measles with the Student Health Service. International students with "alien" status are also required to provide  evidence of freedom from tubercular disease. (All other students will be strongly encouraged to provide results of a T.B. test accomplished within one year of first enrollment at ISU.)

Students who register only for designated off-campus courses through Continuing Credit Extension or register through the International Studies Office for exchange programs at off-campus sites are exempted from these requirements.

Employees who enroll for classes, were born on or after January 1, 1957, and who did not attend ISU classes before July 1, 1989, are required to comply with student immunization requirements.

Proof of Immunity May be Documented By:

  1. A copy of the student's Illinois high school health record which complies with the immunization requirements; or
  2. Other acceptable medical record signed by a physician or public health official showing required immunization dates; or
  3. Completion of immunization section on the ISU Student Health and Immunization Report.

Proof of Immunity Will Not Be Required If:

  1. A licensed physician (M.D. or D.O.) certifies that an immunization(s) is medically contraindicated (Note: If a student's condition later changes to permit immunization, he/she shall be required to obtain the immunization(s) from which he/she had been exempted). OR
  2. The University accepts a written statement by the student detailing his/her objection to immunization based upon bona fide religious tenets or practices. General philosophical or moral objection to immunization shall not be sufficient for an exemption on religious grounds.

If no proof of immunization or certificate/statement of exemption is provided by Labor Day (if entering fall semester) or January 28 (if entering spring semester), the student will not be permitted to register for a subsequent session at ISU and a $25 administrative compliance fee will be assessed.

Students who obtain an exemption from immunization requirements on medical or religious grounds may be excluded from classes if an epidemic occurs at ISU.

Students will be required to file a medical history with the Student Health Service on or prior to their first use of the Student Health Center. An ISU Student Health History and Immunization Report form is mailed to all new students admitted to the University.
